Apex trigger on OpportunityLineItem hits CPU limit at 200 rows, test coverage is 68% and deploys keep failing. Patch it or rewrite the trigger layer?

CPU timeout at exactly 200 rows means someone is doing per-row work inside the bulk chunk - usually a SOQL or a loop-in-loop that goes quadratic. Profile with debug logs at FINEST before rewriting; the fix is often collecting IDs into a map and querying once. But 68% coverage with failing deploys tells me the real problem is no trigger handler pattern - one trigger per object, logic in a testable handler class, and coverage stops being a lottery. Paste the trigger body and the slowest test class and I'll show you where the CPU actually goes.
