Three recurrences means you've been treating symptoms - and 'the temps did it' is a 5-why that stopped one why too early: why do temps make the error? Because the process runs on tribal knowledge, which is a system cause, not a people cause. DMAIC it properly: define the defect in numbers (rate per 100 orders, cost per event), measure where in the flow it enters, analyze with the temps in the room - operators always know the workaround nobody documented. The fix gets a control plan: a poka-yoke in the entry screen beats a memo every time, plus a 90-day control chart so reversion shows in week 2, not month 6.
