4 against an expected 2.5 fails a Kupiec test at most confidence levels, so start there before assuming it's just bad luck. A 250-day lookback is thin for tail risk, and if those breaches clustered around one volatility event that's a model specification problem, not noise. I'd check whether the breaches were independent or clustered - clustering points to a correlation assumption that broke, not just fat tails. Send me the breach dates and I'll run the independence test alongside the Kupiec.
