Your problem is inverted risk - if 90% flow through as standard, the standard catalog was stuffed with things that were never repeatable, pre-approved work, and CAB rubber-stamping 40 items an hour reviews nothing. Rebuild from the risk model: assessment questions that score change collision, blackout windows against the CMDB, and standard templates re-certified quarterly - anything without 10 clean executions loses the badge. Your 2 outage changes will turn out misclassified; run them through the new scoring as the test. CAB then reviews 8 items properly instead of 40 badly.
