The compensation leak is first, no contest - that is an HR criteria failure, and one exposure can cost you the workforce's trust in the whole platform. Audit the COE security: HR criteria on the compensation service, restricted case types for executives, and check whether someone granted a broad HR role instead of scoping by criteria - the usual culprit is one over-permissive role assignment from go-live week. Then the stalls: lifecycle events with owner-less tasks need escalation rules and a fallback assignee after 48 hours, or one absent manager freezes a whole onboarding. Both fixes are days, not weeks.
