Stop swapping embeddings and measure retrieval in isolation - build a 50-question eval set with known source docs and check recall@5. If the right chunk isn't in the top 5, generation never had a chance; if it is, your problem is prompting, not retrieval. A 20% wrong-answer rate usually decomposes into bad chunking - fixed 512 tokens cutting tables in half - plus no reranker; hybrid BM25+dense with a cross-encoder typically moves recall@5 from 0.6 to 0.85. Share 10 failing questions and I'll name your failure mode.
