Fix is async end to end - no .Result, no .Wait(), no GetAwaiter().GetResult() outside true sync entry points - enforced by analyzers and a banned-API list so it can't come back. Finding them: dotnet-counters shows the queue climbing; a dump's thread stacks name every blocked call site, usually 5-10 of them. EF Core's sync query APIs count too.
