The 'runs as admin' finding blocks launch - Extend apps should run against granular domain security with app-specific groups scoped to exactly the custom objects and APIs they touch, never a broad admin ISU. Rebuild that first; reviews rarely accept a scope reduction promised for later. The prod timeout with clean sandbox runs is external API latency plus serial orchestration steps - parallelize the 3 calls where no dependency exists, add per-step timeouts with graceful degradation, and cache static data like catalogs. Sandbox latency lies; measure each call in prod. Send the orchestration definition and I'll mark what runs parallel and what stays serial.
